Advance Care Planning

Do you have your end of life wishes sorted out?

Advance care planning is a process of communication that starts with an important conversation with your loved ones about your hopes, fears, and wishes as it pertains to end-of-life.

Completing an advance directive provides you with an opportunity to share preferences for care as well as select an agent who can express healthcare decisions for you if you are unable to speak for yourself.

Fort HealthCare, Rainbow Hospice Care, and Watertown Regional Medical Center are currently participating in a special advance care planning initiative called Honoring Choices Wisconsin. Together, our three organizations have trained 50 certified advance care planning facilitators – people with the skills to guide you through this important conversation and having the time to meet with you whenever and wherever it’s most convenient.

This is a free service and to date, more than 500 such conversations have taken place locally.
