Currently, Rainbow Hospice Care serves patients and families residing throughout all of Jefferson County, as well as parts of surrounding counties, more specifically, communities in eastern Dane, northern Walworth, southern Dodge, western Waukesha, north-eastern Rock, and south-eastern Columbus counties. Our main office is located in Jefferson, WI—home base for most clinical and nonclinical staff. Note that most patients and their families receive hospice care in their own homes—which may include nursing homes, assisted living facilities, and other residential arrangements. In addition to home care, Rainbow Hospice Care operates its own freestanding inpatient facility in Johnson Creek for those who need a more intensive level of care than can be provided at home. The location of the inpatient facility is ideally located and easily accessible to all those residing in our service area. RHC also has a small administrative office in Whitewater for Chuck Frandson, President of the Rainbow Hospice Care Foundation.
